Please read through the following documents and answer the questions at the bottom of the form. After submitting this form, a pastor or elder will meet with you personally to discuss membership.

If you have any additional questions, please note them in the form and someone will contact you as soon as possible.

1. I Don’t Believe in God

2. I believe there is a God but I have too many questions about God to do more than just acknowledge God may exist

3. I believe that Jesus lived a life that changed history

4. I believe that Jesus said he was God

5. I believe that Jesus said he came to save people from sin

6. I believe that I am a sinner

7. I believe that without Jesus’ forgiveness and cleansing, my sin prevents me from having a relationship with God.

8. I believe that my sin is beyond my ability to make up for on my own so that I can not earn God’s forgiveness and enter into a relationship with God based on the good things I try to do.

9. I believe that if people die without having their sins forgiven they will spend eternity apart from God in Hell.

11. I believe that Jesus gave his life to pay the consequences for my sin that God’s justice demanded and that because he did that he has the right to offer me the forgiveness I need to have a relationship with God.

12. I believe that once I have accepted Jesus' invitation to have my sins forgiven and I enter into a relationship with God, I am His forever and my relationship with God cannot be broken.

13. I believe that Jesus died for my sins and was buried, but that he rose again 3 days later conquering sin and death and is alive today in heaven.

14. I have accepted Jesus’ gift of forgiveness for my sins and have entered into a relationship with God. (If so give the approximate date)
